Creating a Digital Psychoeducation Programme for bipolar disorder in the COVID-19 pandemic

Introduction
The Covid-19 pandemic profoundly affected delivery and accessibility of mental health care services at a time when most needed. The OPTIMA Mood Disorder Service, a specialist bipolar disorder service, adapted group psychoeducation programme for delivery on-line.
ObjectivesWe report the feasibility of creating a digital psychoeducation programme.
MethodsThe OPTIMA ten session group psychoeducation programme was converted into a ‘Digital’ intervention using video-conferencing. Sessions offered a range of key topics, derived from the initial Barcelona Group Psychoeducation Programme. At the time of writing, OPTIMA had fully completed two 10 session digital courses.
ResultsA total of 12 people (6 in each group) consented to be part of a service evaluation of the digital groups. Just over half of the participants were women (7/12; 58.3%) and one identified as being non-binary (8.3); remaining participants were men. Age of participants ranged from 25 years to 65 years (Mean=42.3; SD=13.1). Data showed a high level of engagement (77%) All participants reported some improvement with a mean Bipolar Self-Efficacy scale (BPSES) post-group score of 105.6 (SD=14.8). At group level, this change was not statistically significant (F (1, 15) = 0.71, p=0.41). At an individual level, two out of five showed a reliable change index >1.96.
ConclusionsDelivering a ‘digital’ group psychoeducation programme was possible due to careful planning and programme development. There was good uptake from service users suggesting it is a feasible approach with preliminary evidence of clinical benefit.

Summary
Two competing impulses around gender have come to characterize Protestant life: one that insists on a particular God-ordained gender order, often starting with the home and moving outward to church and society and another that downplayed or sometimes altogether dismissed gender injunctions and hierarchies as contrary to divine intention. Protestantism’s “modernist–fundamentalist” divide, usually seen as a far-reaching dispute over how to read the Bible, is closely connected to and driven by wider cultural debates regarding gender. Gender has functioned as one of the most active organizing forces in Protestant life. Protestants drew on gender to control behaviors and regulate boundaries as well as to question and challenge them. The sectarian nature of Protestantism as it grappled with gender shaped Protestant theologies, rearranged alliances, and splintered institutions.

Outbursts of subglacial water from numerous alpine glaciers have been observed and documented. Such events tend to occur in spring and are thus attributed to an inability of the winter subglacial drainage system (characterized by high water pressure and low capacity) to accommodate a sudden and profuse influx of surface meltwater. Prior to a release event, bursts of glacier motion are common, and the release then precipitates the restoration of summer plumbing that damps or terminates surface acceleration. The events bear witness to the importance of interactions between surface melt, runoff, en-glacial water storage and internal routing, in addition to subglacial drainage morphology. Using a distributed numerical model to simultaneously solve surficial, englacial and subglacial water-transport equations, we investigate the role of these components in a hydro-mechanical event observed at Trapridge Glacier, YukonTerritory, Canada, in July 1990.
Cropping system intensification: vegetable pea can replace fallow between rainfed monsoon rice and irrigated spring rice

The Eastern Gangetic Plain is among the world's most intensively farmed regions, where rainfed and irrigated agriculture coexist. While the region and especially Bangladesh is a major producer of rice (Oryza sativa L. ssp. indica), there is potential to further develop sustainable rice production systems. Specifically, there is scope to include a replacement crop for the short fallow between rice crops in the dominant cropping pattern of rainfed monsoon rice harvest followed by irrigated spring rice. The aim of the current research was to identify a suitable cool-season legume crop – pea (Pisum sativum L.) or lentil (Lens culinaris Medik. ssp. culinaris) – that could be grown in the brief period between rice crops. The study comprised four crop sequence experiments comparing legume cultivars differing in maturity grown in between both long and short duration rice cultivars. These experiments were done at the Bangladesh Rice Research Institute regional station at Rajshahi over three cropping cycles. This was followed by an evaluation of pea vs. fallow between rice crops on three farmers’ fields in one cropping cycle. Here it is demonstrated that green pod vegetable pea is one of the best options to intensify the rainfed monsoon rice–fallow–spring irrigated rice cropping system, notwithstanding other remunerative rabi cropping options that could displace boro rice. The inclusion of an extra crop, pea as green pod vegetable, increased farm productivity by 1·4-fold over the dominant cropping sequence (rice–fallow–rice) and farm net income by fourfold. The study highlighted the advantages in total system productivity and monetary return of crop intensification with the inclusion of a pea crop between successive rice crops instead of a fallow period.

OBJECTIVE
Large-scale studies evaluating risk factors for Clostridium difficile infection (CDI), a leading cause of infectious diarrhea among patients undergoing stem cell transplantation (SCT), are lacking. We have evaluated risk factors for CDI among both autologous SCT (auto-SCT), and allogeneic SCT (allo-SCT) recipients using the National Inpatient Sample (NIS) database provided by the Healthcare Cost and Utilization Project (HCUP).
METHODSWe used patient data obtained from the NIS database for all adult patients admitted for auto- and allo-SCTs from January 2001 to December 2010. We performed multivariate logistic regression analyses to evaluate risk factors of CDI in auto- and allo-SCT patients.
RESULTSAuto-SCTs constituted 61.5% of all SCTs performed during the study period. Of the 53,072 auto-SCT patients, 5.8% had CDI, whereas 8.5% of 33,189 allo-SCT patients had CDI. Univariate analyses identified age, gender, indication for SCT, radiation as part of the conditioning regimen, respiratory failure, septicemia, lengthy hospital stay, and multiple comorbidities as risk factors for CDI in both subsets. On multivariate analyses for auto-SCT, there was significant correlation between age and the indication for transplant (P=.003), but the indication for either auto- or allo-SCT was not associated with CDI on multivariate analyses. The following factors were found to be associated with CDI: septicemia (auto-SCT odds ratio [OR],=1.64; 95% confidence interval [CI], 1.35–2; and allo-SCT OR, 1.69; 95% CI, 1.36–2.1), male gender (auto-SCT OR, 1.29; 95% CI, 1.09–1.53; and allo-SCT OR, 1.36; 95% CI, 1.18–1.57), lengthy hospital stay (auto-SCT OR, 2.81; 95% CI, 2.29–3.45; and allo-SCT OR, 2.63; 95% CI, 2.15–3.22), and presence of multiple comorbidities (auto-SCT OR, 1.32; 95% CI, 1.11–1.57; and allo-SCT OR, 1.18; 95% CI, 1.0–1.4).
CONCLUSIONSThe prevalence of CDI was higher among patients undergoing allo-SCT. CDI was significantly associated with longer hospital stay, septicemia, and male gender for auto- and allo-SCT recipients. While this analysis did not permit us to directly ascribe the associations to be causative for CDI, it identifies the more vulnerable population for CDI and provides a rationale for the development of more effective approaches to preventing CDI.

Measurements of ice thickness and surface elevation are prerequisite to many glaciological investigations. A variety of techniques has been developed for interpretation of these data, including means of constructing regularly gridded digital elevation models (DEMs) for use in numerical studies. Here we present a simple yet statistically sound method for processing ice-penetrating radar data and describe a technique for interpolating these data onto a regular grid. DEMs generated for Trapridge Glacier, Yukon Territory, Canada, are used to derive geometric quantities that give preliminary insights into the underlying basin-scale hydrological system. This simple geometric analysis suggests that at low water pressures a dendritic drainage network exists that evolves into a uniaxial morphology as water pressure approaches flotation. These predictions are compared to hydraulic connection probabilities based on borehole drilling.

Japanese knotweed is an aggressive alien species in Europe, North America, and Australia, causing a range of environmental problems. Eradication of Japanese knotweed is proving to be a difficult task, because the plant is able to propagate generatively by intra- and interspecific hybridization, and vegetatively from shoot and tiny rhizome pieces. Despite the economic consequences of Japanese knotweed on natural and built environments, its physiology is not yet fully understood; especially important are sink-source relations between old and young parts of the rhizome and growth of lateral and latent rhizome buds. Current methods of chemical control include three types of phloem-mobile herbicides, such as glyphosate, imazapyr, and synthetic auxins. These herbicides have limitations on their use, and all fail to eradicate the plant completely, for the reasons discussed in this review. Our aim is to suggest prospective approaches to enable chemical eradication: use of signals to induce controlled growth and development of quiescent rhizome buds; use of phytohormones, sugars, and light to increase allocation of phloem-mobile herbicides to the rhizome; use of xylem-mobile herbicides to exterminate the old rhizome parts; and use of different phloem-mobile herbicides at different growth stages.

The cropping systems of the Eastern Gangetic Plains of Bangladesh, India and Nepal are based on rice. There is a scope to intensify such systems through diversification with lentil, the most popular food legume. Two strategies were evaluated to fit lentil into the short fallow between successive monsoonal (i.e., T. aman) and pre-monsoonal (aus) or irrigated rice (boro) crop. These were early-flowering sole-cropped lentil and relay-sown lentil into rice. Firstly, 18 early-flowering lentil lines at three contrasting sowing dates were tested over two seasons on a research station at Ishurdi in Bangladesh. Secondly, relay sowing was evaluated at the same location with six early-flowering lines and two control cultivars in two seasons. It was also assessed on ten farms in Western Bangladesh, comparing relay with sole cropping over 3 years. Flowering in the early-flowering lentil lines was consistently 9–17 days earlier, than the control cultivars, but they did not achieve an economic yield (<1·0 t/ha). Relay sowing with an existing cultivar produced an economic yield of lentil, which was comparable or higher than sole-cropped lentil in all situations. The relay-sown lentil matured in sufficient time to allow the land to be prepared for the succeeding rain-fed rice crop. It was concluded that the substitution of relay-sown lentil for fallow in the monsoonal rice–fallow–rain-fed rice cropping pattern is a useful option to intensify and diversify cropping in the Eastern Gangetic Plain.
Psychometric validation of a knowledge questionnaire on micronutrients among adolescents and its relationship to micronutrient status of 15–19-year-old adolescent boys, Hyderabad, India

Objective
To develop a psychometrically valid questionnaire for testing knowledge on micronutrients and to assess the relationship between knowledge and biomarkers of micronutrient status among adolescents.
DesignCross-sectional, institution-based, validity and reliability study.
SettingSeven higher secondary schools were covered in the limits of Greater Hyderabad Municipal Corporation, Hyderabad, India.
SubjectsStudents aged 15–19 years, n 92 for the pre-test, n 108 for test–retest and n 109 for studying the relationship between knowledge and biomarkers of Fe, retinol, ascorbic acid, α-tocopherol, folic acid and vitamin B12 status.
ResultsFrom an item pool of 106, thirty-one items were selected based on content validity. Statistical tools to obtain a valid and reliable questionnaire among adolescent boys and girls resulted in eighteen items with a difficulty index of 0·11–0·86, discrimination index of 0·20–0·72 and validity index (point bi-serial correlation) of 0·10–0·62. Reliability as measured by Cronbach's α was 0·71 and the intra-class correlation coefficient was 0·80. A Bland–Altman plot showed good agreement between test and retest scores. The mean response score to the eighteen-item questionnaire was 5·2 (sd 2·68). The mean values of serum retinol were significantly different (P = 0·022) between groups below (24·8 (sd 6·64) μg/dl) and above (28·0 (sd 7·67) μg/dl) the 50th percentile of knowledge score. The relationship persisted after controlling for economic status as a covariate using analysis of covariance (P = 0·018). Other micronutrients did not show any significant relationship.
ConclusionsA valid and reliable eighteen-item knowledge questionnaire was constructed and found to have a significant positive relationship with plasma retinol status alone.

A model for predicting the various transformations undergone in the soil by nitrogen applied in farm wastes, in response to variations in soil and climatic factors, is presented. The soil is divided into layers and a simple mass balance model is used to describe the movement and redistribution of water within the soil profile, as a function of rainfall, evapotranspiration and soil moisture characteristics.
Ammonification of native and applied organic N is assumed to follow first-order reaction kinetics and nitrification and denitrification are treated as zero-order reactions. The rates or rate constants for all three reactions were related separately to soil temperature, moisture content and pH.
Uptake of nitrogen by a grass crop, dry-matter production, removal of dry matter and N through harvests, addition to soil of C and N through dead plant material and stubble and the consequent immobilization of mineral N are allowed for. Some of the predictions of the model are compared with the results of a lysimeter experiment.

Sodic soils are widespread, especially in the Indo–Gangetic plain. Amelioration with gypsum is effective, especially when combined with growing a crop of rice. However, it has proved difficult to generate new varieties of sodic-tolerant rice, because of the difficulties of screening – other than in the field, where spatial variation of sodicity is notoriously high. Growing plants at high pH in solution culture in a controlled environment is problematic, because of the effects of pH on nutrient availability. This study evaluated a system of growing rice in a nutrient solution with added Tris but without minor nutrients, which were supplied in a foliar spray. Performance was evaluated from the change in weight and in appearance and correlated with performance in soil. Growing rice in the presence of Tris (4 mM, pH 8·6) provides a means of evaluating genotypic differences in response to sodicity.
